Key Responsibilities

• Conduct thorough evaluations that assess patient history, perform specific assessments, and document results clearly and accurately.
• Discuss evaluation findings and proposed treatment plans with patients and their families, ensuring full engagement in goal-setting processes.
• Implement individualized treatment protocols rooted in evidence-based practices while adapting to each patient's unique presentation and healthcare needs.
• Collaborate with interdisciplinary teams, recognizing changes in a patient's condition that may necessitate modifications to care plans or require further medical consultations.
• Manage discharge planning and follow-up care meticulously, ensuring all documentation, including initial evaluations and progress notes, is timely and clear, providing a comprehensive account of patient care.
• Effectively manage your schedule, meet productivity targets, and participate actively in team meetings and community outreach initiatives.
• Contribute to departmental goals and compliance with healthcare regulations, fostering an environment of continuous improvement.
• Initiate professional development by pursuing continuing education opportunities, serving as a mentor to students or new staff.
• Promote exceptional patient experiences by offering flexible access to care, listening to feedback, and engaging in initiatives aimed at improving service delivery.

Patient Population / Client Focus

The primary patient population includes individuals requiring rehabilitative care in homes ranging from post-operative recovery to chronic pain management. As a Physical Therapist, you will be instrumental in tailoring therapy interventions to suit the needs of older adults, patients with mobility limitations, and individuals recovering from surgery or injury.

Qualifications

• A Bachelor's Degree in Physical Therapy is required.
• A valid Physical Therapy License (DPT) for the State of New Jersey.
• Current certification in Basic Life Support (BLS) from the American Heart Association is mandatory.
• Strong interpersonal and communication skills that promote effective collaboration with patients and healthcare teams.

Preferred Experience

• At least two years of experience in a physical therapy setting, preferably within home care or outpatient environments.

Compensation and Benefits
