Work Environment

This position is primarily onsite, as you will be traveling to patients' homes in a designated territory to provide therapy services. Key Responsibilities

• Conduct in-home evaluations to assess patients' physical conditions and develop comprehensive care plans tailored to their individual needs.
• Implement therapeutic interventions such as exercises for strength, mobility training, gait analysis, and balance exercises to facilitate recovery.
• Educate patients and their caregivers about treatment plans and provide guidance on home exercise programs to ensure continuity of care.
• Work collaboratively with interdisciplinary teams, including physicians, nurses, and occupational therapists, to enhance patient outcomes through coordinated care.
• Document patient progress accurately and promptly, adhering to Medicare requirements and our agency's standards of care.
• Travel efficiently within your designated territory to meet patients in their homes, managing your schedule effectively to maximize care delivery.

Patient Population / Client Focus

The patients you will work with are primarily adults and seniors requiring rehabilitation services due to various conditions, such as post-surgical recovery, neurological disorders, chronic pain, and mobility impairments. Your personalized approach will be crucial in helping them regain independence and improve their overall well-being.

Qualifications

• Must hold a valid Physical Therapist (PT) license in the state of New Jersey.
• Graduate from an accredited Physical Therapy program with a Doctor of Physical Therapy (DPT) or equivalent degree.
• A minimum of two years of post-education experience in the field of physical therapy.
• Strong communication and organizational skills are essential for effective patient interactions and documentation.
• Ability to work autonomously as well as collaboratively with other healthcare professionals in a homecare environment.

Preferred Experience

While a minimum of two years of experience is required, experience in home health care settings is highly preferred. Candidates with a background in geriatric therapy, neurological rehabilitation, or post-operative care will find their skills particularly applicable.

Compensation and Benefits

Clinica Recovery LLC offers a competitive compensation package ranging from $110,000 to $125,000 annually, along with mileage reimbursement for travel. Additional benefits include:
