Work Environment

This is a full-time, onsite position focusing on home care, where you will interact directly with clients in their living environments. Our team operates Monday to Friday from 9 AM to 5 PM. This structure allows for consistent patient interaction, fostering deeper relationships and better understanding of each client's specific challenges and progress.

Key Responsibilities

• Conduct thorough physical assessments for each client to develop comprehensive and tailored therapy plans that reflect their goals and limitations.
• Design and implement progressive therapy programs aimed at improving mobility, relieving pain, enhancing balance, and overcoming functional limitations.
• Provide hands-on therapeutic treatments, including exercises, gait training, and fall prevention strategies, adapted to the unique environment of each client's home.
• Assist clients in regaining independence in their daily activities by promoting confidence and safety within their living space.
• Educate clients, caregivers, and family members about therapeutic exercises and wellness strategies to ensure continuity of care and progressive improvement.
• Maintain accurate and detailed documentation in accordance with healthcare standards, ensuring the seamless transition and continuity of care.
• Collaborate with nursing staff, caregivers, and fellow therapists to provide holistic care that addresses the total well-being of each client.
• Monitor patient progress closely, making necessary adjustments to treatment plans and communicating any noteworthy changes to family and physicians.

Patient Population / Client Focus

Your primary focus will be on a diverse client base that primarily includes older adults and individuals recovering from injuries or surgeries. Your empathetic communication skills and tailored therapy plans will be crucial in addressing the unique mobility and functional challenges faced by each client. By establishing rapport and trust, you will facilitate not just recovery but overall wellness.

Qualifications

• Active Physical Therapist license in New Jersey is mandatory.
• A Bachelor's degree in Physical Therapy from an accredited institution is required.
• A minimum of two years of hands-on experience in physical therapy is essential.
• Strong clinical acumen, excellent communication skills, and the ability to adapt interventions to changing patient needs are a must.
• Prior experience working with geriatric populations or in assisted living settings is highly preferred.
• CPR and First Aid certification is preferred; additional certifications in geriatrics or neurological rehabilitation are advantageous.
• A valid driver's license and reliable transportation to visit clients at home are essential for this role.

Preferred Experience

• Experience working in home health or a similar setting is strongly preferred.
• Familiarity with electronic medical records (EMR) and other documentation tools relevant to home health therapy services.
• Demonstrated ability to work independently while managing a diverse client caseload effectively.

Compensation and Benefits
