Work Environment

In this position, you will primarily work in an outpatient office setting, transitioning to patients' homes within your designated territory. You will have a flexible Monday to Friday schedule from 9 AM to 5 PM, allowing you to manage your time effectively while engaging with clients in various residential settings. This role requires a high degree of mobility as you travel to different locations, ensuring that your therapeutic services are accessible to all patients.

Key Responsibilities

• Conduct thorough in-home assessments to evaluate patients' physical conditions, functional limitations, and recovery objectives.
• Develop and implement individualized treatment plans that utilize evidence-based interventions aimed at restoring mobility, strength, and daily living skills.
• Provide specialized therapeutic exercises, gait training, balance retraining, and conditioning programs designed to meet the specific goals of each patient.
• Educate patients and their caregivers on effective treatment protocols, self-management strategies, and home exercise programs to enhance recovery.
• Collaborate with physicians, nurses, and other healthcare professionals to coordinate comprehensive patient care and ensure consistent progress towards recovery milestones.
• Maintain accurate and timely documentation of patient assessment results, treatment sessions, and progress notes, adhering to Medicare and agency standards for quality care.
• Travel regularly to patients' homes within your assigned geographic area, fostering a strong community presence and building lasting relationships with clients.

Qualifications

• Must be a licensed Physical Therapist (PT) in the state of New Jersey.
• A graduate of an accredited Physical Therapy program with a Bachelor's degree (DPT or equivalent).
• Two years of experience in physical therapy is required, specifically involving direct patient care.
• Valid driver's license and reliable transportation for travel to patient locations.
• Strong communication, interpersonal, and time management skills necessary to provide effective patient education and care.
• Ability to work independently and manage time efficiently while providing high-quality home care services.

Preferred Experience

• Experience in home health care or rehabilitation services is highly desirable.
• Familiarity with electronic medical records (EMR) systems to streamline documentation and patient management.
• Background in developing community health programs or working with interdisciplinary teams would be advantageous.

Compensation and Benefits

We offer a competitive salary ranging from $110,000 to $125,000 annually, commensurate with experience and qualifications, along with mileage reimbursement for travel expenses. Our comprehensive benefits package includes:
