Work Environment

This full-time position is on-site, allowing you to directly interact with residents in their home care setting. You will typically work Monday through Friday from 9 AM to 5 PM, engaging with clients in a comfortable and familiar environment that promotes healing and recovery. Our facility emphasizes a collaborative culture among interdisciplinary teams, including nursing staff and other therapy disciplines, ensuring comprehensive care for our residents.

Key Responsibilities

• Conduct thorough physical therapy evaluations for residents to assess their physical capabilities and limitations.
• Develop and implement personalized treatment plans focused on enhancing mobility, balance, strength, and overall functional ability.
• Deliver one-on-one therapy sessions, utilizing evidence-based practices to address specific patient needs and goals, such as improving gait and reducing pain.
• Continuously monitor and document the progress of each patient, reassessing and modifying treatment plans as necessary based on evolving needs.
• Work closely with nursing staff and caregivers to coordinate care and ensure a holistic approach to each resident's rehabilitation.
• Educate residents and their families on effective physical therapy techniques, body mechanics, and fall prevention strategies to empower them in their own recovery journeys.
• Maintain timely and accurate documentation of therapy services in adherence to company policies and regulatory requirements, ensuring high standards of care.

Patient Population / Client Focus

In this role, you will primarily focus on the geriatric population, catering to adults aged 40 years and older who require ongoing therapeutic interventions. Many of these residents may be dealing with chronic conditions, post-surgical rehabilitation needs, or the effects of aging that impact their physical independence. Understanding the unique challenges faced by this demographic will be essential in providing effective patient-centered care.

Qualifications

• Active Physical Therapist license in the state of New Jersey is required.
• A degree from an accredited Physical Therapy program (DPT, MPT, or equivalent).
• Strong communication skills to effectively collaborate with residents and their families.
• Adept at developing and managing treatment plans that prioritize individual patient goals.
