Work Environment

This position is onsite, primarily focused on providing services in the homes of our patients located in and around Fort Lee, New Jersey. The role will involve direct interactions with patients in their living environments, offering a unique opportunity to assess their needs comprehensively. Each day, you will engage with a variety of challenging and rewarding cases that require creative problem-solving and adaptability.

Key Responsibilities

• Conduct thorough physical therapy evaluations to assess each patient's physical capabilities and limitations.
• Develop and implement individualized treatment plans tailored to meet each resident's specific needs and therapy goals.
• Deliver one-on-one therapy sessions aimed at enhancing patient mobility, strength, balance, and pain management through evidence-based techniques.
• Regularly monitor patient progress and modify treatment plans as necessary to ensure optimal outcomes.
• Engage in collaborative practice with nursing staff, caregivers, and other healthcare professionals to provide holistic care.
• Educate patients, families, and caregivers on effective body mechanics, injury prevention strategies, and mobility techniques to foster independence.
• Maintain meticulous and timely documentation compliant with both company standards and regulatory requirements.

Patient Population / Client Focus

The majority of our clients are seniors aged 40 to 65 years, many of whom have specific needs related to conditions such as arthritis, previous surgeries, and mobility impairments. Your role as a Physical Therapist will be crucial in improving their physical functioning, thereby positively impacting their daily lives. Through individualized care, you will play an integral part in their rehabilitation journey, helping them achieve greater independence and better health outcomes.

Qualifications

• An active New Jersey Physical Therapist (PT) License is mandatory.
• A Bachelor's Degree in Physical Therapy from an accredited program is required (Doctorate or Master's preferred).
• Demonstrated strong interpersonal and communication skills are essential for effectively engaging with patients and families.
• Ability to work independently in patient homes while also being an effective member of an interdisciplinary team.
• A compassionate, patient-centered approach to care that prioritizes the well-being of the residents.

Preferred Experience

• At least 2 years of experience as a Physical Therapist, preferably within geriatric or assisted living settings.
• Previous experience in home health care is advantageous but not required.

Compensation and Benefits
