This full-time, onsite position focuses on delivering high-quality physical therapy services directly within clients' homes, promoting enhanced mobility, health, and independence. Work Environment

This role operates primarily within a home care setting, allowing you to meet clients in their own environments. Key Responsibilities

• Develop customized physical therapy treatment plans aimed at preventing disability, alleviating pain, and restoring muscle function, ensuring alignment with each client's unique capabilities and the prescribed medical treatment plan.
• Supervise and oversee Physical Therapy Assistants to ensure the effective implementation of care protocols and maintain high standards of service delivery.
• Conduct comprehensive evaluations of clients' functional abilities through diagnostic testing of muscles, nerves, joints, and overall functional capabilities.
• Assess home environments to identify obstacles to rehabilitation and recommend modifications that enhance client independence in daily activities.
• Deliver physical therapy services in accordance with physician directives, including manual exercises, functional activities, and physical agent modalities such as traction and massage.
• Evaluate and properly fit clients with prosthetic and orthotic devices, collaborating with physicians to suggest necessary modifications.
• Prepare thorough clinical documentation, including initial evaluations, care plans, progress notes, and discharge summaries, maintaining compliance with OASIS and other regulatory standards.
• Participate actively in interdisciplinary team meetings, contributing to holistic treatment planning and client evaluations.
• Engage with clients and their families, providing education and support regarding therapy programs and the use of physical therapy devices.
• Actively contribute to the continuous improvement of patient care by completing additional assignments and fulfilling duties as requested.

Patient Population / Client Focus

In this role, you will work with a diverse range of clients, including individuals recovering from surgery, those managing chronic conditions, and elderly patients transitioning to independent living. Your ability to provide compassionate, personalized care will be essential to improving clients' overall quality of life and physical health.

Qualifications

• A Doctorate in Physical Therapy (DPT) from an accredited institution is required.
• Current and valid physical therapist licensure in New Jersey.
• One year of practical experience as a physical therapist in either acute care or home care settings.
• Current CPR certification to ensure patient safety during therapy sessions.
• Strong decision-making skills and willingness to accept responsibility for patient care decisions.
• Excellent communication and interpersonal skills, with a focus on patient and family engagement.
• Ability to employ creative problem-solving techniques to address complex patient situations.
• Commitment to ongoing professional development and adhering to ethical clinical practices.

Preferred Experience

• Experience with OASIS documentation and familiarity with Medicare guidelines.
• Background in developing community resource utilization plans to enhance client support.
• Leadership experience within an interdisciplinary healthcare team setting.

Compensation and Benefits

We offer a competitive salary ranging from $100,000 to $120,000 annually, based on experience and qualifications.
