Work Environment

This position is categorized as onsite and will involve direct engagement with patients in their home settings. You can expect to work approximately 40 hours each week, with a variable schedule that may include weekends. Your role will be essential in creating a personalized and comfortable treatment atmosphere for patients, as home care allows for tailored interventions in environments that promote healing and recovery.

Key Responsibilities

• Conduct thorough evaluations and assessments of new patients to identify their specific needs and limitations.
• Develop individualized treatment plans, setting measurable goals that align with each patient's rehabilitation process.
• Implement effective physical therapy interventions, regularly reassessing patient progress and modifying treatment plans as necessary.
• Provide education to patients and their families on treatment goals, exercises, and self-management strategies for long-term recovery.
• Maintain accurate and timely documentation of patient interactions, treatments provided, and progress reports in compliance with applicable laws and professional standards.
• Collaborate with a multidisciplinary team to enhance patient care, including communication with physicians and other healthcare providers regarding treatment plans and updates.
• Engage in community outreach and continuous quality improvement initiatives, enhancing our presence and impact in the local community.
• Participate in regular staff meetings, trainings, and continuing education to enhance your skills and stay updated on best practices.

Patient Population / Client Focus

You will work closely with a diverse patient population, encompassing individuals with various orthopedic and neurological conditions, post-operative recovery needs, and geriatric clients facing mobility challenges. The ability to interact with patients from different backgrounds and with varying levels of health literacy is crucial, as you will need to adapt your communication and treatment strategies accordingly to promote effective recovery.

Qualifications

• Bachelor's Degree in Physical Therapy from an accredited institution is required.
• A valid state license to practice physical therapy in New Jersey.
• Strong written and verbal communication skills to ensure effective interactions with patients, families, and colleagues.
• Proficient in word processing and documented computer skills, essential for maintaining patient records and compliance with documentation standards.

Preferred Experience

A minimum of two years of clinical experience in physical therapy is preferred, with an emphasis on home health or outpatient settings being an asset. Familiarity with electronic health records (EHR) systems will also be beneficial.

Compensation and Benefits

Healthactions Physical Therapy offers a competitive salary range of USD 100,000 – 120,000 annually, commensurate with experience and qualifications.
