Home Health Physical Therapist - Full-Time Opportunity in Englewood, NJ

This full-time role is instrumental in providing personalized physical therapy services to patients in the comfort of their own homes, primarily in Englewood, New Jersey. Work Environment

As a Home Health Physical Therapist, you'll work onsite, primarily visiting patients in their residences. This position allows for a flexible work schedule from Monday to Friday, 9 AM to 5 PM, enabling you to balance your professional and personal life. You will travel within your designated territory to provide in-home assessments and treatments, ensuring each patient receives the individual attention and care they deserve.

Key Responsibilities

• Conduct thorough in-home assessments to evaluate patients' physical conditions, including mobility, strength, coordination, and endurance.
• Develop and implement individualized treatment plans based on patient assessments, addressing specific conditions and aims to enhance recovery.
• Administer therapeutic exercises and skilled interventions such as gait training and balance retraining to aid in the rehabilitation process.
• Educate patients and their caregivers on therapy goals, home exercise programs, and effective techniques for daily living.
• Collaborate with a multidisciplinary team including physicians, nurses, and occupational therapists to ensure comprehensive care and optimal patient outcomes.
• Maintain meticulous and timely documentation in line with Medicare and agency standards, tracking patient progress and treatment efficacy.
• Manage travel to and from patient homes efficiently, ensuring all appointments are met without delays.

Patient Population / Client Focus

In this role, you will work with a variety of patients, including those recovering from surgery, individuals with chronic conditions, and elderly clients requiring assistance to regain independence. The focus is on enhancing both physical function and emotional well-being, creating a supportive environment that fosters healing and confidence.

Qualifications

• Active and valid Physical Therapist (PT) license in the state of New Jersey is required.
• Bachelor's degree in Physical Therapy or an equivalent from an accredited institution.
• Demonstrated competence in physical therapy practices, with excellent communication and interpersonal skills.
• Ability to work independently and manage time efficiently in a home care setting.
• Strong organizational skills and attention to detail for maintaining patient records.

Preferred Experience

• A minimum of two years of physical therapy experience is required; home health experience is highly advantageous.
• Background in geriatric care and familiarity with chronic health conditions.
• Experience working within a multi-disciplinary healthcare team.
