This position offers a competitive salary range from USD 90,000 to 100,000 per year.

Key Responsibilities\n

As a Licensed Clinical Social Worker at CHDC, your day-to-day responsibilities will include but are not limited to:

• Providing clinical behavioral health services that align with the mission and quality standards of the health center, ultimately enhancing patient experiences and outcomes.

• Aiding mental health consumers in navigating both physical and behavioral healthcare systems to ensure they receive fully integrated care tailored to their unique needs.

• Educating patients about available mental health resources and engaging them in wellness programs that foster disease prevention and promote healthy lifestyles.

• Actively participating in multi-disciplinary team meetings to discuss patient cases, provide insights, and collaborate on treatment plans.

• Promptly reporting all crises and unusual incidents to the Medical Director, with immediate escalation of psychiatric emergencies to the Chief Medical Officer, ensuring the highest level of patient safety and care.

• Completing thorough documentation for clinical records, maintaining accurate and up-to-date patient information as required.
\n
• Engaging in the Quality Improvement program, attending all associated meetings and training sessions to continually elevate the level of care provided.
\n
• Utilizing evidence-based screening tools to assess patient health needs and report findings for optimal care planning.
\n
• Specializing in the treatment of patients with Opioid Use Disorder, collaborating effectively with our Center of Excellence Team to deliver integrated care.

Patient Population / Client Focus\n

In this role, you will primarily serve hospice patients and their families, focusing on those with complex, chronic conditions requiring compassionate and expert care. Your work will significantly impact the quality of life for these patients, providing them with the emotional and psychological support necessary during critical phases of their healthcare journey.

Qualifications\n

To be successful in this role, you should meet the following qualifications:

• A Master’s degree in Social Work from an accredited institution.

• Current and valid Licensed Clinical Social Worker (LCSW) credentials.

• At least 2 years of direct experience in the field, with a strong foundation in mental health services.

• Experience in a medical setting, such as primary or specialty care, is preferred, demonstrating an ability to work collaboratively with a diverse range of healthcare professionals and support staff.

• Willingness to travel to various CHDC locations for meetings, workshops, conferences, and outreach activities is essential.

Preferred Experience\n
