Key Responsibilities\n

• Engage in effective communication with physicians and other healthcare providers to report on patient progress, needs, concerns, and any potential or actual issues that arise during treatment.

• Maintain detailed and accurate records, including clinical assessments and progress notes, in accordance with the clinic’s policies and best practices for documentation.

• Collaborate with interdisciplinary team members to develop comprehensive treatment plans based on thorough psychosocial assessments, ensuring continuity of care that includes discharge planning when necessary.

• Provide individual and group counseling, crisis intervention services, and emotional support to patients during challenging situations, focusing on constructive coping strategies for emotional and behavioral problems.

• Assist clients with discharge preparations and care coordination, connecting them to necessary community resources to facilitate their continued recovery process.

Patient Population / Client Focus\n

Qualifications\n
