Key Responsibilities\n

• Assist primary care providers in recognizing and managing patients' mental health and psychosocial issues, effectively acting as a vital member of the healthcare team.

• Conduct detailed patient intakes, focusing on diagnostics, evaluations, and making informed recommendations regarding treatment plans to primary care providers.

• Provide brief therapeutic interventions tailored to individual patients' needs and engage in follow-up communications to assess progress and adapt treatment if necessary.

• Develop and maintain relapse prevention plans that empower patients to stabilize their mental health and functioning.

• Monitor and coordinate the delivery of health services for behavioral health care, ensuring that patients receive comprehensive care both within and beyond our clinic.

• Document patients’ progress accurately in treatment charts, maintaining clear communication with primary care providers about their needs and achievements.

• Educate patients on self-management strategies and provide vital information to enable active participation in their recovery journey.

• Collaborate with clinic management on behavioral health services, contributing insights for program evaluation and improvement.

Qualifications\n

• A minimum of a Master’s Degree in Clinical Counseling.

• Valid and current LPC (Licensed Professional Counselor) license in Texas.

Compensation and Benefits\n
