Key Responsibilities\n

In your role as a Licensed Professional Counselor, you will engage in various critical functions to promote mental health and well-being. Your responsibilities will include but are not limited to:

• Providing brief, targeted interventions for patients requiring mental health support.

• Collaborating with primary care providers to develop comprehensive treatment plans that align with established agency guidelines and Federally Qualified Health Center (FQHC) protocols.

• Initiating follow-up conversations to monitor patient progress, making necessary adjustments to treatment approaches based on ongoing assessments.

• Creating relapse prevention plans for patients, equipping them with tools to maintain stable functioning and preventing the exacerbation of mental health conditions.

• Documenting each patient’s progress and diagnostic information meticulously in treatment charts to ensure compliance and comprehensive care tracking.

• Advising on patient referrals to specialized mental health facilities when necessary, while monitoring the linkage between health services.

• Offering educational support and self-management strategies to empower patients in their recovery journey.
