Key Responsibilities\n

• Conduct detailed psychosocial evaluations of patients, assessing their needs, abilities, and behaviors through interviews and direct observations.

• Provide individual and family therapy to address a variety of mental health challenges, including anxiety, depression, end-of-life issues, substance abuse, and chronic pain.

• Utilize evidence-based interventions and therapeutic techniques to facilitate healing and coping strategies for patients and their families.

• Engage in complex discharge planning, working closely with healthcare teams to ensure seamless transitions from hospice care to home, while providing essential consultations and education for families.

• Create and maintain accurate records in compliance with regulatory standards, documenting patient progress and care plans effectively.

• Promote connections within the community by associating patients and families with additional supportive services, resources, and organizations.

Patient Population / Client Focus\n

The primary clientele includes patients receiving hospice care, many of whom face terminal illnesses and significant emotional distress. You will play an essential role in alleviating psychological burdens not only for patients but also for their families, guiding them through grief, uncertainty, and the complexities of end-of-life decisions. Qualifications\n

• A Master’s degree in Social Work (MSW) from an accredited institution.

• Valid and active California state license as a Licensed Clinical Social Worker (LCSW).

• A minimum of two years of direct experience in a clinical setting, preferably in hospice or palliative care.

• Strong understanding of mental health challenges faced by diverse populations, particularly those facing life-limiting conditions.

• Exceptional communication skills, both verbal and written, with the ability to build rapport with patients and team members.

Preferred Experience\n

Compensation and Benefits\n
