Vascular Surgery APP Opportunity

Expected start date ASAP. Provider Type: APP Specialty Vascular Surgery FTE needed: 1.0. Daily Shift: Standard 12-hour and/or 24-hour call shifts. Expected duration of assignment: 6 months. Reason for opening: Gap coverage.

Schedule/Shift hours:

• 12-hour call shifts 0700-1700 OR 1700-0700 (this would be beeper) Monday-Friday
• 24-hour call shifts 0700-0700 Monday-Friday
• 24-hour call weekend

We would like to have a weekday 12-hour call rate with a 4 hour gratis, beeper fee, a weekday 24-hour rate with a 6 hour gratis and then a weekend 24-hour rate with a 4 hour gratis. Clinic hours are 0800-1630. Although these are call shifts, the APP needs to report at the beginning of their shift if it's a 0700 start time to do a handoff, rounding, etc.

Current gaps are night shifts for the below dates:

• October - 3, 4
• November - 5, 25, 26, 27
• December - 12, 13, 16, 24, 28, 31

Type of privileges (clinic/hospital/both): Both. Setting: Inpatient, Outpatient or Both? Both. If Inpatient, # of beds: 36. Call requirement: 6 shifts a month. Rounding, admissions, or none? Both.

Patient demographics: Vascular/Endovascular. Average patients per day: 15-20. Required Procedures: First assist in OR and assist with in clinic procedures. Required clinical activity in the last 24 months (case logs): Applicant must provide documentation of provision of clinical services representative of the scope and complexity of the privileges requested during the previous 24 months. First, Second or Third Assist in General Surgery and Surgical Sub-Specialties: Must provide documentation of training and competency. Case log of surgical cases for last 24 months is required along with documentation by surgeon noting whether assistance was as first, second or third assist. The NP privilege, "First Assist in Surgery": Must provide specific documentation of training and competency including type and number of procedures in the last two years as well as the name and contact information for the primary operator.

Will provider work solo? No, there will be a MD/DO they will be working with. Licensed Candidate: We would prefer OR licensed in order to make those October and/or November dates, but we are open to non-OR for later dates. Required certifications (Must be from American Heart Association or American Red Cross as applicable): BLS, ACLS. EMR: EPIC.
