Key Responsibilities\n

• Coordinate and implement home visits, ensuring thorough observations, chart reviews, and data analysis to monitor client progress.

• Actively participate in Interdisciplinary Team Meetings and other essential discussions, contributing insights to optimize clinical care.

• Document monthly and quarterly progress notes that succinctly capture patient developments and treatment effectiveness.

• Organize training for staff members and colleagues to ensure the highest standards in program implementation and patient care documentation.

• Conduct follow-up interviews and additional home visits, providing ongoing support to individuals beyond initial treatment.
