MSW Social Worker, PACE Case Manager (Bilingual)

FLORIDA PACE CENTERS, INC.

Miami Jewish Health is seeking a Social Worker MSW for its PACE program. The role provides direct social work case management, including participant screening, case management, counseling, and referrals for participants in the MJHS PACE Program. Responsibilities include interdisciplinary assessments, care planning, documentation, and liaison with community agencies to advocate for participants.
