Key Responsibilities\n

• Conduct comprehensive assessments of psychosocial needs to determine suitable treatment strategies, improving individual care plans through informed decision-making.

• Provide case management and short-term counseling, including crisis intervention, tailored to meet each patient’s unique circumstances.

• Assist patients in navigating complex benefit systems, including entitlements and supplemental insurance, to reduce reliance on emergency assistance programs.

• Facilitate access to prescription cost assistance by connecting patients with pharmaceutical assistance programs and community resources.
\n
• Serve as a patient advocate in communications with community agencies, ensuring clients receive appropriate support and services.
\n
• Maintain precise and timely patient records, preparing necessary documentation and reports that contribute to ongoing treatment strategies and compliance.

• Coordinate transportation services for patients in need, addressing barriers to care and promoting accessibility.
\n
• Engage in regular clinical and team meetings to discuss case progress and collaboratively enhance treatment approaches.

Patient Population / Client Focus\n

Our patient population encompasses a wide range of mental health challenges, including but not limited to anxiety, depression, trauma-related disorders, and substance use issues. You will work with individuals from diverse backgrounds and age groups, tailoring interventions to align with each client’s distinct needs and cultural context. Qualifications\n

• A Master’s degree in Social Work, Psychology, Marriage and Family Therapy, or Counseling is required for this position.

• The candidate must hold an active and independent license: LPCC, LCSW, or LMFT.

• A minimum of two (2) years of professional experience in a mental health or clinical counseling setting.

Preferred Experience\n

While not mandatory, experience in outpatient mental health practice, familiarity with telehealth platforms, and previous advocacy work within community support systems are highly advantageous. Compensation and Benefits\n
