Key Responsibilities\n

• Diagnose and treat a variety of mental, emotional, and behavioral disorders, adjusting treatment plans according to patient progress and needs.

• Conduct thorough psychosocial assessments to identify the specific needs of patients and develop tailored care plans.

• Refer patients to essential community resources, treatment programs, support groups, or specialists, ensuring a continuum of care.

• Collaborate with multidisciplinary teams to develop and implement effective discharge plans, aimed at ensuring patients transition smoothly to ongoing care.

• Organize and prioritize daily tasks while managing multiple patients' needs, including new intakes, ongoing therapy, and discharge planning.

• Provide social work consultation and crisis intervention as necessary, including bereavement support and psychosocial strategies.

• Actively engage with outside agencies, insurers, and healthcare providers to facilitate comprehensive care for clients.

• Support fellow team members as needed, providing debriefing and assistance in the aftermath of adverse events or challenging cases.
