Key Responsibilities\n

• Conduct comprehensive ongoing assessments to accurately determine and address the individual needs of each patient.

• Maintain clear and open communication with supervisors, direct care staff, and family members to update on patient progress.
\n
• Create and share a tentative schedule with supervisors detailing appointments, home visits, and client observations to monitor and evaluate treatment effectiveness.

• Coordinate and conduct home visits as part of in-home observations, ensuring compliance with treatment plans and performing chart reviews.
\n
• Guarantee the effective implementation of program plans in collaboration with team members and ensure necessary adjustments are made based on patient progress.

• Participate actively in Interdisciplinary Team Meetings to discuss patient care strategies and outcomes.
\n
• Prepare comprehensive monthly and quarterly progress notes to document patient advancement.
\n
• Organize training sessions to empower staff and ensure they are competent in implementing program plans and documentation protocols.

• Provide follow-up support through interviews and home visits to ensure continued patient care.

• Maintain detailed records and documentation that are readily accessible for review by pertinent parties.

• Act as a liaison with public stakeholders and the mental health community to facilitate joint goals and share resources.

• Contribute to the creation of training materials aimed at improving program efficiency and effectiveness.
