Key Responsibilities\n

• Conduct comprehensive evaluations and assessments to understand the psychosocial needs of clients and their families.

• Actively participate in the development and implementation of Individualized Family Service Plans (IFSPs) to ensure tailored support for each client.
\n
• Monitor and evaluate client outcomes and progress, adjusting care plans as necessary to reflect changing needs.
\n
• Provide guidance and emotional support to families, enhancing their coping strategies during challenging times.
\n
• Utilize evidence-based practices to develop joint strategies with families that promote well-being and stability.
\n
• Implement Natural Learning Environment Practices to facilitate the learning and development of children with disabilities.
\n
• Collaborate with a multidisciplinary team to coordinate services that align with regulatory compliance and organizational goals.
\n
• Adhere to all federal and state regulations to maintain high-quality care standards within the hospice setting.

Patient Population / Client Focus\n

The primary client focus for this role includes children with disabilities or delays, infants, and toddlers, as well as their families. By addressing these children’s unique physical, cognitive, emotional, and communication needs, you will play a crucial role in supporting their development and quality of life. Additionally, you will work with parents and guardians to ensure they have the necessary resources and tools for effective caregiving during their loved one’s end-of-life journey.

Qualifications\n

• Master's Degree or Doctorate in Social Work required.

• Current licensure as a Licensed Clinical Social Worker (LCSW) from the Department of Public Health.

• A valid driver's license is necessary for travel to client locations when needed.
\n
• Proficient in adapting therapeutic techniques to suit individual learning styles.

• Strong organizational skills and the ability to manage multiple cases systematically.
\n
• Excellent communication skills to foster trust and rapport with clients and team members.

• Respectful and empathetic, with the ability to collaborate effectively within a team environment.

Preferred Experience\n

• A minimum of one year’s experience working within early intervention systems or similar settings focused on child development.

Compensation and Benefits\n
