This position operates in a fully remote setting that allows LCSWs the flexibility to impact clients' lives effectively while maintaining a work-life balance. Our hospice center focuses on providing early intervention services, assisting young children with disabilities and developmental delays, ensuring they and their families have the necessary resources and support for growth and development.

Key Responsibilities\n

• Conduct thorough evaluations and assessments to identify each child's strengths and needs.

• Participate actively in the development and implementation of Individualized Family Service Plans (IFSP) that guide interventions.
\n
• Monitor treatment outcomes collaboratively with other team members to ensure effective service delivery.
\n
• Build strong, trusting relationships with children and families to create a supportive and nurturing environment.
\n
• Provide evidence-based recommendations to families, empowering them with strategies aligned with the Natural Learning Environment Practices.
\n
• Utilize a coaching model that enhances learning and development within the context of natural settings.
\n
• Ensure compliance with all federal, state, and local regulations as well as organizational policies to maintain the highest standards in service quality.

Patient Population / Client Focus\n

The ideal candidate will work primarily with infants and toddlers, focusing on those with disabilities or developmental delays. Recognizing the family as a central unit of support, the LCSW will guide family members in navigating resources and implementing strategies to foster their child's development, covering areas such as cognitive, social/emotional, and communication skills.

Qualifications\n

• A Master’s Degree in Social Work or a Doctorate of Social Work from an accredited institution.
\n
• Current licensure as a Licensed Clinical Social Worker (LCSW) in compliance with the Department of Public Health regulations.

• A valid driver’s license is required for potential in-person family meetings and assessments.
\n
• Ability and confidence to adapt therapeutic interventions to various learning styles and individual needs.

• Strong organizational and time management skills to handle multiple cases efficiently.

Compensation and Benefits\n
