Work Environment\n

This position is a fully remote opportunity, allowing for flexible work arrangements that cater to both your professional and personal life. While the primary schedule is Monday to Friday from 8:00 AM to 4:30 PM, you may occasionally need to provide support during evenings or weekends based on client needs. Successful candidates will be comfortable with the flexibility required to ensure continuous care and are prepared to be on-call for urgent situations that arise outside normal working hours.

Key Responsibilities\n

• Conduct comprehensive assessments of clients' psychosocial needs, utilizing detailed methodologies that address emotional, mental, and social factors effectively.

• Collaborate closely with clients and their families to formulate personalized care plans tailored to their specific goals and preferences, ensuring a client-centered approach.

• Provide counseling and emotional support to clients, particularly seniors, navigating significant life transitions such as grief, loss, and changes in health.

• Act as a strong advocate for clients, helping them access essential community resources and support services that enhance their overall quality of life.

• Work as part of an interdisciplinary team, including other healthcare professionals and caregivers, to coordinate care and deliver holistic services that address all aspects of a client's well-being.

• Conduct routine visits to assess and monitor clients' living conditions, wellbeing, and potential safety issues, ensuring they remain in supportive environments.

• Facilitate positive communication among family members involved in the care of senior clients, resolving conflicts and enhancing family dynamics.

• Educate clients and their families about local community resources, support groups, and programs to boost social engagement and mental health.

• Maintain accurate and timely documentation of all client interactions, care plans, and assessments as mandated by best practices and organizational policies.

Patient Population / Client Focus\n

The primary focus of this role is on senior clients and their families, who may be dealing with a range of psychosocial challenges, including depression, anxiety, and the complexities of aging. Your understanding of aging-related issues will be pivotal in providing the right support and interventions that empower these individuals to thrive in their daily lives.

Qualifications\n

• A Master's degree in Social Work from an accredited institution, accompanied by current state licensure as a Licensed Clinical Social Worker (LCSW).

• A minimum of two years of experience in a social work role, ideally within a community health or geriatric setting.

Preferred Experience\n

Compensation and Benefits\n
