Key Responsibilities

• Collaborate with attending consultants and other healthcare providers to develop and implement individualized patient care plans.
• Evaluate and document patients’ reactions to prescribed care, ensuring that adjustments are made based on their progress.
• Conduct telephone consultations and manage laboratory follow-ups while triaging walk-in patients to appropriate care providers.
• Diagnose and prescribe treatments based on thorough assessments, utilizing evidence-based guidelines for decision-making.
• Serve as a clinical resource for other healthcare providers and non-medically trained staff, offering guidance on best practices.
• Participate in mentoring and interviewing new nurse practitioners, helping to foster a nurturing work environment.
• Engage in the ongoing development, review, and revision of agency-specific nursing practice guidelines.
• Audit peer practices to maintain high quality standards in patient care.
• Support quality assurance initiatives and incorporate findings into everyday clinical practices.
• Ensure patient confidentiality is maintained while providing direct patient care clinical sessions.
• Conduct comprehensive health histories and perform problem-specific examinations tailored to patients’ presenting concerns.
• Order and interpret diagnostic tests, managing both acute and chronic health issues in accordance with approved protocols.
• Develop and implement patient care plans that consider various treatment modalities, including medications, nutrition, exercise, and traditional nursing interventions.

Patient Population / Client Focus

The nurse practitioner will cater to a diverse patient population, addressing both acute and chronic health issues. You will be instrumental in improving patient outcomes by providing tailored, compassionate care to individuals and families in the Newburgh area, working with patients from various backgrounds.

Qualifications

• A Bachelor’s Degree in Nursing is required; a Master’s Degree in Nursing is preferred.
• A valid and unrestricted New York State Nurse Practitioner license, with certification in Family Health or Adult Health.
• Current New York State Registered Nurse license is preferred.
• A current NPI number and DEA certification are essential for this role.

Preferred Experience

• A minimum of three years of clinical experience as a Nurse Practitioner, preferably in outpatient or ambulatory care settings.
• Experience working collaboratively in a multidisciplinary healthcare team.
• Proficiency in using electronic health record systems for efficient patient documentation and communication.
