Work Environment\n

This is a remote position based in Plano, Texas, allowing for flexibility while maintaining a strong connection to our dedicated team. Team members are supported in their roles with access to telehealth platforms and other digital resources, fostering collaboration and continuous communication. We offer a structured workweek from Monday to Friday, adhering to a standard schedule of 9:00 AM to 5:00 PM to maintain work-life balance.

Key Responsibilities\n

• Act as a primary liaison between families, clients, and outpatient providers, ensuring that everyone involved has a clear understanding of care plans and progress.

• Engage with insurance companies to navigate pre-certification processes and manage utilization effectively, advocating for the best possible care for our clients.

• Participate in discharge and aftercare planning to ensure a smooth transition for clients leaving our care.
\n
• Offer therapeutic meal support during sessions, modeling a healthy relationship with food and addressing any related concerns or issues clients may face.
\n
• Provide safe, empathetic, and affirming care tailored to the unique needs of each client, being sensitive to their intersecting identities and life experiences.

Patient Population / Client Focus\n

This role primarily focuses on working with older adults who are in hospice care, addressing both their mental health needs and those of their families during a critical time. Our clients often face complex emotional challenges, and as an LCSW, you will support them through grief, anxiety, and other concerns related to their life stages while advocating for their dignity and autonomy.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution.

• A valid Licensed Clinical Social Worker (LCSW) license in the state of Texas.

• Experience working specifically with older adults is essential for understanding the unique needs of this population.

• Strong clinical assessment skills and the ability to diagnose and create appropriate treatment plans.

• Exceptional communication and interpersonal skills to facilitate effective collaboration with families and multidisciplinary teams.

Preferred Experience\n

Compensation and Benefits\n
