Key Responsibilities\n

• Conduct thorough assessments of patients' psychosocial needs, identifying immediate and long-term treatment goals.

• Provide case management services, offering short-term counseling and emergency crisis intervention as necessary.

• Assist patients in navigating complex systems to complete entitlement forms, access supplemental insurance, and obtain community benefits that reduce reliance on emergency services.

• Facilitate access to pharmaceutical assistance programs, guiding patients through the process of obtaining prescription cost support.
\n
• Act as a patient advocate, effectively liaising with community agencies and resources to ensure comprehensive service delivery.

• Maintain accurate and up-to-date electronic patient records, preparing detailed reports and documentation on patient interactions and progress.

• Coordinate transportation services for patients when necessary to facilitate their access to care.

• Engage in regular clinical meetings to discuss cases, share insights, and collaborate on best practices for treatment.

Patient Population / Client Focus\n

You will work with a diverse client base, including individuals facing a variety of mental health challenges such as anxiety, depression, and relationship issues. Qualifications\n

• A Master’s degree in Social Work, Psychology, Marriage and Family Therapy, or Counseling is required.

• Valid and active licensure as a Licensed Professional Clinical Counselor (LPCC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT) in California.

• Minimum of two (2) years of professional experience in mental health counseling or a related field.

Preferred Experience\n

Compensation and Benefits\n
