Key Responsibilities\n

• Conduct comprehensive assessments to identify the mental health needs of each patient, ensuring tailored therapeutic interventions.

• Maintain open communication with supervisors, caregivers, and families regarding patient progress and treatment plans.

• Develop and share a tentative schedule for appointments, home visits, and therapeutic observations to optimize service provision.

• Coordinate home visits for in-home evaluations and support, including chart reviews and staff interviews to gather relevant data.

• Ensure that treatment plans are implemented effectively and adjusted based on continuous assessments and feedback from patients and caregivers.

• Engage in regular Interdisciplinary Team Meetings to discuss patient cases and collaborate on care strategies.

• Complete detailed monthly and quarterly progress notes that track patient development and treatment efficacy.

• Organize training sessions to certify team members in implementing treatment plans and accurate documentation practices.

• Conduct follow-up interviews and home visits to ensure ongoing support for each patient’s evolving needs.
