Key Responsibilities

As a Physical Therapist at Lakeland Care Solution, you will be responsible for:

• Conducting thorough evaluations of patients referred by physicians to identify functional impairments and establish appropriate treatment programs.
• Designing and implementing evidence-based therapeutic interventions tailored to individual patient needs, ensuring alignment with the physician's plan of care.
• Providing care in both individual clinical settings and home health environments, utilizing a variety of treatment modalities, including manual therapy techniques and therapeutic exercises.
• Maintaining clear and effective communication with patients, their caregivers, and interdisciplinary teams to maximize recovery outcomes.
• Documenting treatment sessions accurately in our electronic health record (EHR) system to meet compliance and quality assurance standards.
• Contributing to the overall effectiveness of the clinic by meeting minimum utilization goals and supporting business objectives.

Qualifications

The ideal candidate will possess the following qualifications:

• A Bachelor's degree in Physical Therapy from an accredited program.
• A state license in Physical Therapy.
• Basic Life Support (BLS) certification from the American Heart Association.

• A commitment to building meaningful relationships with patients and leveraging those connections to improve therapy outcomes.

Preferred Experience

While the minimum requirement is two years of experience in the field, candidates with additional experience, especially in home health, pediatric, or sports medicine therapy, will be highly regarded. A Doctorate of Physical Therapy (DPT) and Board Certification in various specialties are preferred but not mandatory. Candidates should also hold a current CPR certification.

Compensation and Benefits
