Work Environment

This role is primarily onsite, as you will be visiting patients in their homes throughout Lehigh Acres and the surrounding areas. The work environment is dynamic and unique, requiring adaptability as you navigate different home setups and patient needs. You will collaborate with an interdisciplinary team, comprising physicians, social workers, and family members, to ensure a comprehensive approach to patient care.

Key Responsibilities

• Conduct thorough assessments of patients with orthopedic impairments, including pain management, mobility challenges, and recovery from surgical procedures.
• Evaluate patient histories and gather relevant data through interviews and diagnostic tools, ensuring a holistic understanding of each patient's medical background.
• Design and implement individualized treatment plans tailored to each patient's specific needs and goals, utilizing evidence-based practices.
• Provide direct therapy interventions, including exercises, manual therapy techniques, and modalities to promote healing and functional independence.
• Monitor patient progress, adjusting treatment plans as necessary based on ongoing assessments and patient feedback.
• Educate patients and their families on home exercises, safety precautions, and strategies to enhance recovery outside of therapy sessions.
• Maintain accurate documentation of patient care, progress notes, and billing information per regulations and organizational policies.
• Ensure compliance with state practice acts and standards, while also adhering to Life and Wellness Services policies to uphold the highest care standards.
• Participate in regular team meetings to discuss patient outcomes and share insights that can improve service delivery.

Patient Population / Client Focus

The patient population for this position includes individuals recovering from various orthopedic conditions, such as joint replacements, fractures, and sports-related injuries. You will work with patients of all ages, tailoring your interventions to meet the developmental and rehabilitative needs specific to each person. Your role is instrumental in enhancing their recovery journey and helping them reclaim their physical abilities.

Qualifications

• A Bachelor's degree or higher from a CAPTE-accredited program in physical therapy.
• Current Physical Therapist license in the State of Florida, with a temporary permit being acceptable.
• Certification in Basic Life Support (BLS) for healthcare providers, ensuring readiness for emergency situations.

Preferred Experience

• Previous experience in orthopedic rehabilitation, preferably within a home care setting.
• Familiarity with electronic health records (EHR) systems to streamline documentation and improve workflow efficiency.
• Strong communication skills to effectively educate patients and collaborate with interdisciplinary team members.

Compensation and Benefits
