Work Environment

This role is primarily onsite as a home health Physical Therapist, requiring travel to patients' residences within the Rego Park area. Working in diverse environments allows for a tailored therapeutic experience, fostering strong interpersonal relationships and enabling clinicians to make real-time adjustments in treatment plans based on patient feedback and progress.

Key Responsibilities

• Conduct comprehensive evaluations of patients referred by physicians to develop and implement personalized treatment plans.
• Utilize a variety of evidence-based therapeutic approaches to address conditions across various specialties, including orthopedic rehabilitation, pediatric therapy, neurology, and sports medicine.
• Provide education and training to patients and their caregivers, empowering them with tools and knowledge to enhance recovery.
• Adhere to physician-determined plans of care, ensuring that all rehabilitation goals are consistently met and documented using our electronic health record (EHR) system.
• Monitor patient progression and adapt treatment protocols to optimize outcomes, while ensuring compliance with best practices.
• Deliver exemplary customer service throughout all interactions, contributing to a positive health care experience and fostering trust and rapport with clients.
• Collaborate with interdisciplinary teams to enhance patient care and share insights on treatment methodologies.
• Meet established business objectives and utilization goals, including patient visit targets.

Patient Population / Client Focus

Our Physical Therapists work with a diverse patient population, including individuals recovering from surgical procedures, managing chronic pain, and requiring rehabilitation from sports injuries. Qualifications

• A Bachelor's degree in Physical Therapy, obtained from an APTA or CAPTE-accredited program.
• Current state licensure in Physical Therapy to practice in New York.
• Basic Life Support (BLS) certification from the American Heart Association.
• Strong verbal and written communication skills to effectively convey treatment plans and engage with patients and their families.
• Proficient in using computer systems and EHRs for managing patient documentation and schedules.
• A committed approach to building and maintaining relationships, ensuring each patient receives individualized care that aligns with their functional goals.
• A genuine desire to connect with patients and enhance their quality of life through therapeutic interventions.

Preferred Experience

• Experience in home health care delivery or a community-based setting is strongly preferred.

Compensation and Benefits

We offer a competitive annual salary ranging from USD 100,000 to 120,000, commensurate with experience and qualifications.
