Key Responsibilities\n

• Conduct thorough psychosocial assessments of patients and families, identifying emotional, social, and environmental factors that may impact their health and coping mechanisms related to their illness.

• Develop, implement, and evaluate individualized treatment plans incorporating diverse social work interventions, ensuring they address patients' unique needs and circumstances.

• Collaborate closely with healthcare providers, including physicians and nursing staff, to create comprehensive discharge plans that facilitate a smooth transition from care settings and involve the patient and family in decision-making processes.

• Provide education on coping strategies and caregiving assistance to patients and their families through counseling sessions that focus on adaptation to changes in health status.

• Utilize crisis intervention techniques and deliver both brief and long-term therapeutic modalities, including individual, group, and family therapy, as appropriate.

• Act as a consultant to fellow staff members, sharing expertise in psychosocial oncology and enhancing the overall care environment.

• Engage with consulting physicians and third-party payers to streamline discharge planning and facilitate patient navigation through healthcare systems.

• Evaluate and address mental health concerns, especially in oncology patients, through resources such as screening for depression and anxiety, coupled with short-term counseling.

• Participate in after-hours on-call rotations to provide support and guidance for urgent cases, ensuring continuity of care.

• Lead educational sessions and conferences, presenting as an expert in psychosocial oncology practice, contributing to professional development and staff training initiatives.

Qualifications\n

• A Master's degree in Social Work (MSW) from an accredited institution.
