Key Responsibilities\n

  Conduct thorough assessments to understand clients' psychological and emotional needs, leveraging tailored questionnaires and interviews to identify issues and goals.

  Document session notes meticulously in our secure electronic health record (EHR) system, ensuring compliance with all regulatory guidelines and maintaining confidentiality.

  Engage in collaborative discussions with clients about their treatment journey, encouraging self-advocacy and participation in their care.

  Maintain ethical standards and confidentiality in all client interactions, while adhering to state and federal regulations.

Patient Population / Client Focus\n

Your primary focus will be on clients within the hospice setting, helping individuals and families cope with end-of-life issues, emotional distress, and the psychological aspects of terminal illness. You will support these clients through grief, loss, and transitions while providing empathetic guidance during some of the most challenging times in their lives.

Qualifications\n

  A Master’s degree in Social Work from an accredited institution.

  Current and active state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n
