Key Responsibilities\n

• Provide individualized therapy and counseling sessions to clients both in groups and one-on-one.

• Conduct thorough psychosocial assessments to understand each patient’s unique challenges and strengths, followed by developing customized treatment plans to address identified issues.

• Implement evidence-based therapeutic techniques, including Cognitive Behavioral Therapy (CBT), Dialectical Behavioral Therapy (DBT), and trauma-informed care, to effectively address diverse client needs.

• Offer crisis intervention and case management services, including resource referral and advocacy to ensure patients receive necessary support.

• Document all patient interactions meticulously in our EHR system, ensuring compliance with HIPAA regulations and maintaining the highest standards of confidentiality.

• Participate in clinical meetings and case reviews, collaborating with psychiatrists, psychologists, and other healthcare providers to coordinate comprehensive care.

• Engage in ongoing professional development to enhance clinical skills and stay current with best practices in social work and behavioral health care.

Patient Population / Client Focus\n

This role primarily focuses on working with patients under hospice care, addressing emotional and social factors related to terminal illness. Qualifications\n

• Active LCSW license in California, with no current disciplinary actions.

Compensation and Benefits\n
