Key Responsibilities\n

As a Licensed Clinical Social Worker, you will be entrusted with a wide array of responsibilities aimed at providing therapeutic support and advocacy for both patients and families. Key duties include:

• Conducting thorough clinical assessments to identify psychosocial needs and developing tailored treatment plans that align with best practices in hospice care.

• Implementing various therapeutic modalities, such as cognitive-behavioral therapy (CBT) and grief counseling, to help patients cope with the emotional challenges of terminal illness.

• Maintaining accurate clinical documentation that complies with HIPAA and other regulatory standards to ensure patient confidentiality and care quality.

• Collaborating closely with a multidisciplinary treatment team, including physicians, nurse practitioners, and counseling staff, to create a cohesive treatment approach tailored for each patient.

• Engaging in case consultations and attending peer review meetings aimed at improving treatment plans and enhancing the quality of patient care.

Patient Population / Client Focus\n

In this role, you will primarily work with patients in hospice care who are experiencing a range of mental health issues related to terminal illnesses, including but not limited to anxiety, depression, and grief. You will have the opportunity to engage with patients at one of the most vulnerable times in their lives, making your role crucial for their emotional and psychological support. You will also support their families, guiding them through complex emotional terrains associated with end-of-life issues.

Qualifications\n

• A graduate degree in Social Work from an accredited institution.

• A current and valid license to practice as a Licensed Clinical Social Worker (LCSW) in California.

Compensation and Benefits\n

The salary for this full-time position ranges from USD 115,000 to 130,000 annually, reflective of the experience and qualifications of the candidates. In addition to a competitive salary, we offer:
