Work Environment\n

This position offers a remote work setting, allowing for flexibility to manage your schedule while serving patients in a hospice center. By employing technology such as telehealth platforms and electronic health records (EHR), you will maintain a personal connection with patients and their families while ensuring the efficient management of care. Our full-time schedule runs Monday through Friday from 9:00 AM to 5:00 PM, creating a structured yet adaptable work routine.

Key Responsibilities\n

• Conduct comprehensive assessments of patients' mental health, evaluating emotional, behavioral, and social functioning to determine therapeutic needs.

• Develop and implement individualized treatment plans that leverage evidence-based strategies tailored to the unique circumstances of each patient in hospice care.

• Provide psychotherapy addressing issues such as grief, anxiety, depression, and adjustment to terminal illnesses, helping patients navigate their emotional experiences.

• Monitor patient progress diligently, reviewing treatment efficacy and making necessary adjustments in response to patients’ and families' feedback.

• Educate patients and their families about mental health conditions, emphasizing available treatment options and community resources that may assist during challenging times.

• Collaborate closely with interdisciplinary teams, including physicians, nursing staff, and palliative care specialists, to ensure a holistic approach to patient care.

• Adhere to professional standards, including confidentiality and ethical guidelines, while navigating the complexities of hospice care.

Patient Population / Client Focus\n

The successful LCSW will work with a diverse population of hospice patients, many of whom contend with terminal diagnoses. Your role will involve addressing not only the psychological aspects of terminal illness but also supporting family members who are impacted by the loss. By facilitating conversations around end-of-life care, you will guide patients and families through the emotional labyrinth of their unique situations.

Qualifications\n

• A Master’s degree in Social Work from an accredited institution is required.

• Active Licensed Clinical Social Worker (LCSW) licensure in the state of Arizona.

• A minimum of 2 years’ experience in providing psychotherapy and related mental health services, especially within end-of-life or hospice contexts.

• Strong clinical skills, particularly in conducting assessments and developing culturally competent treatment plans.

• Proficiency in utilizing electronic health records (EHRs) and other digital tools to maintain accurate patient documentation and support care coordination.

Compensation and Benefits\n
