Key Responsibilities\n

• Conduct thorough assessments of patients in hospice care to understand their emotional and psychological needs, including their presenting issues, background, and individualized treatment goals.

• Deliver individual, couples, and family therapy, employing evidence-based modalities such as cognitive-behavioral therapy (CBT), mindfulness strategies, or narrative therapy tailored to the unique experiences of patients and their families.

• Develop comprehensive and personalized treatment plans, constantly adapting interventions based on the evolving needs and preferences of the patients in hospice.

• Engage with patients to monitor progress towards treatment goals and adjust therapeutic strategies as necessary to ensure optimal outcomes.

• Maintain strict confidentiality while adhering to ethical guidelines and legal requirements specific to the practice and social work standards.

Patient Population / Client Focus\n

Qualifications\n

• A Master’s degree in Social Work from an accredited program.

• Active state licensure as a Licensed Clinical Social Worker (LCSW).

Compensation and Benefits\n

This position offers a competitive salary range of $80,000–$104,000 annually, commensurate with experience and qualifications.
