Key Responsibilities\n

• Conduct comprehensive assessments of clients' psychosocial needs, paying close attention to emotional, mental, and social factors that affect their overall well-being.

• Collaborate with clients and their families to create personalized care plans, ensuring that these plans align with individual goals and preferences.

• Provide counseling and emotional support to clients and their families, guiding them through life transitions and the challenges associated with end-of-life care.

• Advocate for clients' rights, ensuring access to community resources, and support systems required for comprehensive care.

• Work alongside nurses, caregivers, and other healthcare professionals as part of an interdisciplinary team to coordinate and enhance quality of care.

• Conduct regular virtual and when necessary, in-person visits to monitor clients’ well-being, assess their living conditions, and intervene when safety issues arise.

• Facilitate effective communication and conflict resolution among family members involved in the senior client's care.

• Educate clients and their families on available community resources, support groups, and programs that enhance social engagement and quality of life.

• Maintain up-to-date and accurate documentation of client assessments, care plans, progress notes, ensuring compliance with ethical and legal standards in social work practice.
