This position is primarily remote, allowing flexibility while working from home with occasional visits to our Fresno, California, facility as needed. Regular teleconferences and virtual meetings will facilitate communication and updates on patient care plans, while ensuring alignment with organizational goals.

Key Responsibilities\n

• Conduct comprehensive psychosocial assessments through interviews, observation, and preliminary review of medical records to evaluate emotional, social, and environmental factors affecting patients' recovery and quality of life.

• Provide individual and group counseling to patients and their families, addressing emotional challenges, adjustment issues, and end-of-life transitions while promoting mental well-being and resilience.

• Collaborate closely with the rehabilitation team to develop and implement individualized treatment plans, incorporating assessment findings that take into account patients' psychosocial needs.
\n
• Guide patients in accessing community resources, including support groups, vocational rehabilitation programs, and housing assistance, facilitating their connection to essential services beyond rehabilitation.
\n
• Educate patients and families regarding available community resources, post-rehabilitation services, and coping strategies, empowering them to navigate life after treatment effectively.
\n
• Actively participate in discharge planning, ensuring a smooth transition for patients and their families while coordinating with external service providers.

Patient Population / Client Focus\n

As a Clinical Social Worker in our hospice care setting, you will work with patients who are facing terminal illnesses, assisting them in processing their emotions and fears while providing essential support for their families. You will engage with individuals from diverse backgrounds, ensuring culturally competent care that respects their values, beliefs, and personal wishes during one of the most challenging times in their lives.

Qualifications\n

• A valid license as a Clinical Social Worker (LCSW) in the state of California.

• A Bachelor’s degree in Social Work or a related field; a Master’s degree in Social Work is preferred.

• A minimum of one year of experience in a clinical social work setting, ideally within rehabilitation or hospice care.
