Key Responsibilities\n

• Conduct thorough assessments and evaluations of clients to identify their emotional, psychological, and social needs, ensuring that appropriate treatment plans are developed and implemented.

• Execute mandated reporting and duty to warn processes effectively, ensuring client safety while adhering to ethical and legal guidelines.

• Collaborate with interdisciplinary healthcare teams to create holistic treatment plans that address both the physical and emotional aspects of patient care.

• Monitor and document client progress, adjusting treatment plans as necessary to reflect their evolving needs and circumstances.

Patient Population / Client Focus\n

The primary focus of this role involves working with a diverse group of clients in a hospice setting. You will provide emotional support to terminally ill patients and their families, addressing concerns related to grief, loss, and mental health issues that often arise during end-of-life care. Qualifications\n

• A Master's degree in Social Work, Counseling, or a related mental health field is required.

• A current and valid LCSW license in California.
