Key Responsibilities\n

• Conduct comprehensive assessments to evaluate patients' psychosocial needs, identifying appropriate treatment and referral pathways.

• Provide case management, short-term counseling, and crisis intervention services to support patients through challenging times.
\n
• Assist patients in navigating and completing applications for benefit programs, entitlements, and supplemental insurance, aiming to reduce dependence on emergency assistance.

• Facilitate access to prescription cost assistance programs through community resources and pharmaceutical companies, ensuring patients receive necessary medications.
\n
• Advocate for patients’ best interests, liaising with community agencies to secure supportive services.
\n
• Maintain accurate and timely documentation of patient records, including preparing reports and written summaries as needed for clinical meetings.

• Coordinate transportation assistance to ensure patients can access appointments and services, enhancing their overall journey toward recovery.

• Participate in clinical team meetings and case conferences to discuss patient progression and support a holistic approach to care.

Patient Population / Client Focus\n

Qualifications\n

• A master’s degree in Social Work, Psychology, Marriage and Family Therapy, or Counseling is required.

• Possession of one of the following active licenses: Licensed Professional Clinical Counselor (LPCC), Licensed Clinical Social Worker (LCSW), or Licensed Marriage and Family Therapist (LMFT).

• Minimum of two years of relevant professional experience in a mental health counseling capacity.
