Key Responsibilities\n

• Conduct thorough assessments of clients during initial evaluations and ongoing sessions to determine their needs and create tailored treatment plans.

• Maintain regular communication with supervisors, direct care staff, family members, and relevant stakeholders to discuss client progress and adjust plans as necessary.

• Provide supervisors with a clear schedule regarding appointments, home visits, and any observations required for monitoring client outcomes.

• Organize and execute home visits to collect observational data, conduct chart reviews, and perform staff interviews that contribute to individualized care strategies.

• Ensure the effective implementation of therapeutic practices as outlined in the treatment plan, monitoring for fidelity and efficacy in real-time.

• Participate actively in Interdisciplinary Team Meetings to promote cohesive care approaches and share insights regarding client development.

• Document monthly and quarterly progress notes that reflect client advancements and plan revisions.

• Facilitate training sessions to ensure all team members are well-versed in program plan implementations and adherence to documentation protocols.

• Provide follow-up support through interviews, home visits, and observations to reinforce therapy and address any evolving client needs.

Patient Population / Client Focus\n

In this role, you will be working with a diverse array of clients, including both adults and adolescents. Qualifications\n

• A Master’s or Doctorate degree in a relevant field such as Psychology, Social Work, or Counseling is required.

• Candidates must hold an active and unrestricted license as either a Licensed Marriage and Family Therapist (LMFT), Licensed Professional Clinical Counselor (LPCC), or Licensed Clinical Social Worker (LCSW) in California.

• A minimum of two years of counseling experience is essential, particularly within a clinical setting.

• A background in family therapy, couples therapy, trauma-informed care, or substance misuse is strongly preferred.

Compensation and Benefits\n
