Key Responsibilities\n

• Collaborate with primary care providers to identify, treat, and manage mental health and psychosocial issues, enhancing overall patient care.

• Conduct thorough patient intakes, focusing on diagnostic and functional assessments, and develop tailored treatment plans in conjunction with primary care providers.

• Provide brief intervention services for patients seeking mental health support, ensuring accessibility of care and resources.

• Communicate effectively with primary care providers regarding patient status, recommending referrals to specialized mental health facilities when necessary.

• Initiate and monitor follow-up care for patients to assess progress and adjust treatment strategies as required.
\n
• Develop relapse prevention plans and assist patients in achieving stable functioning, particularly for those experiencing at-risk behaviors.
\n
• Coordinate health services within the behavioral healthcare spectrum, linking patients to external providers with their consent.
\n
• Maintain accurate and up-to-date documentation of patient progress, diagnoses, and treatment plans in electronic health records.
\n
• Consult with management and other interdisciplinary team members on behavioral health services and program evaluation metrics.
\n
• Support patients in complying with prescribed treatments and navigate medication management, offering coping strategies for side effects.

• Uphold the Code of Ethical Conduct for Clinical Professionals while executing all duties and responsibilities.

Qualifications\n

Compensation and Benefits\n

This position offers a competitive salary range of USD 87,000 to USD 105,000 annually, commensurate with experience and qualifications.
