Key Responsibilities\n

• Conduct comprehensive assessments of clients' psychosocial needs, examining emotional, mental, and social factors that affect their overall health and happiness.

• Collaborate with clients and their families to formulate personalized care plans, aligning with individual goals and preferences while considering their circumstances and challenges.

• Provide empathetic counseling and emotional support for seniors and their families, particularly during life transitions, coping with grief, or managing chronic conditions.

• Advocate for clients' rights and enhance their access to community resources, ensuring they receive the support and services essential for their well-being.

• Engage with interdisciplinary teams, including healthcare professionals and caregivers, to coordinate care efforts and improve client outcomes.

• Conduct routine client follow-ups to monitor their well-being, assess living conditions, and address emerging concerns or safety issues.

• Facilitate effective communication among family members involved in senior care to resolve conflicts and promote a collaborative care approach.

• Educate clients and their families on available community resources, support groups, and programs that can enhance their quality of life and social engagement.

• Maintain meticulous documentation of client assessments, care plans, progress notes, and interactions in compliance with organizational policies.

Qualifications\n

• Master's degree in Social Work from an accredited institution.

• Current and valid state licensure as a Licensed Clinical Social Worker (LCSW).

Fort Behavioral offers a competitive salary ranging from USD 70,000 to 100,000 annually, commensurate with your experience and qualifications.
