Key Responsibilities\n

• Conduct comprehensive assessments by interviewing patients and their families to evaluate social, emotional, and economic factors that may hinder their access to optimal medical care.

• Develop and implement individualized treatment plans that utilize trauma-informed and evidence-based therapeutic techniques, with a focus on substance use disorder and trauma recovery.

• Provide ongoing support through telehealth sessions, ensuring consistent follow-up on client progress and modifying treatment strategies as needed.

• Research and advocate for community resources, such as financial assistance, housing support, and healthcare services, to improve client well-being and enhance their quality of life.

• Effectively respond to crisis situations, including mental health emergencies and instances of child abuse, ensuring appropriate interventions and referrals to safeguard client safety.

• Maintain accurate and timely documentation of therapy sessions, treatment plans, and progress notes, adhering to Department of Mental Health billing and paperwork standards.

• Collaborate with interdisciplinary teams, including medical professionals and care coordinators, to create a holistic approach to patient care.

Patient Population / Client Focus\n

In this role, you will be primarily working with a diverse population that includes individuals facing mental health issues, substance use disorders, and trauma, especially within the context of hospice care. You will engage with clients ranging from children to adults, providing culturally competent care tailored to their unique needs and circumstances.

Qualifications\n

• A Master’s degree in Social Work (MSW) is required.

• A valid Licensed Independent Clinical Social Worker (LICSW) license is essential for practicing in this role.

• At least two years of experience in providing mental health services, preferably in a clinical or health-focused setting.

• Experience in working with Veterans is highly preferred.
\n
• Strong understanding of child development, parenting dynamics, and issues related to child abuse and neglect.

• Exceptional verbal and written communication skills are crucial for counseling and advising clients effectively.

Compensation and Benefits\n
