Position Overview

Peak Performance PT is seeking a dedicated and licensed Physical Therapist for a full-time position within our home care services. This role involves providing exceptional patient care as well as rehabilitation support to a diverse group of clients who require personalized physical therapy in their residence. The Physical Therapist will be instrumental in designing and implementing tailored therapy plans aimed at enhancing mobility, improving patients' quality of life, and promoting overall well-being.

Work Environment

This role is primarily onsite, with Physical Therapists delivering services directly in patients' homes across Long Beach, California. Our working hours are from Monday to Friday, between 2 PM and 8 PM, allowing for flexibility and balance in your schedule. Key Responsibilities

As a Physical Therapist at Peak Performance PT, you will be responsible for:

• Conducting comprehensive patient assessments to evaluate physical limitations, pain levels, and functional capabilities.
• Developing individualized care plans that prioritize patient goals, ensuring a personalized approach to therapy.
• Administering therapeutic interventions and exercises in accordance with each patient's established care plan and the physician's directives.
• Providing education and training to patients and their families regarding therapy exercises, use of assistive devices, and home safety modifications.
• Maintaining accurate and up-to-date patient records, documenting progress and adjustments in care effectively.
• Coordinating with a multidisciplinary team, including physicians, nursing staff, and other therapists, to ensure comprehensive patient care.
• Managing a dynamic caseload, prioritizing clinical tasks effectively, and participating in regular team meetings to discuss patient progress and care strategies.
• Staying current with advancements in physical therapy practices and regulatory requirements, ensuring compliance with state and federal laws.

Patient Population / Client Focus

Your primary focus will be on providing high-quality rehabilitative care to various patient demographics, including postoperative patients, individuals recovering from sports injuries, and patients with chronic pain conditions. Our model emphasizes a hands-on approach that fosters strong patient-provider relationships, as patients often require encouragement and support throughout their recovery journey.

Qualifications

The ideal candidate for this Physical Therapist position will possess:

• A Bachelor's Degree in Physical Therapy.
• Active licensure to practice as a Physical Therapist in the state of California.
• Basic Cardiac Life Support (BCLS) certification or willingness to obtain it within 30 days of employment.
• Empathetic communication skills and the ability to motivate and inspire patients to participate in their rehabilitation.
• A passion for improving the health and lives of others through physical therapy.

Preferred Experience

While no specific prior experience is required, candidates with a Doctor of Physical Therapy (DPT) degree will be given preference. Experience in home health care or outpatient settings will be advantageous, as will familiarity with therapy technologies, mobility aids, and therapeutic modalities.
