Work Environment\n

This role is primarily remote, though it will involve some travel to various CHDC sites for meetings, off-site retreats, conferences, workshops, and outreach functions. Key Responsibilities\n

The primary functions of the Licensed Clinical Social Worker will include:

• Providing clinical behavioral health services that align with CHDC’s mission, enhancing patient outcomes and satisfaction.

• Assisting mental health consumers in effectively navigating both the physical and behavioral healthcare systems to ensure coordinated care.
\n
• Educating clients about available resources and services, fostering participation in programs that promote disease prevention and encourage healthy lifestyle choices.
\n
• Active participation in multi-disciplinary treatment team meetings, contributing insights and strategies for comprehensive patient care.
\n
• Timely reporting of crises and unusual incidents to the Medical Director, with immediate notification to the Chief Medical Officer in the case of psychiatric or medical emergencies.
\n
• Maintaining thorough and detailed clinical documentation for each individual’s records, fulfilling all guidelines set forth by the organization.
\n
• Involvement in quality improvement programs, attending all assigned meetings and training sessions to continuously enhance service delivery.
\n
• Ensuring adherence to organizational policies and procedures, actively working within our integrated healthcare model.
\n
• Utilizing evidence-based screening tools to assess the healthcare needs of individuals and report outcomes accurately.
\n
• Collaborating with our Center of Excellence Team to manage cases of patients with Opioid Use Disorder, ensuring a holistic and evidence-based treatment approach.

Patient Population / Client Focus\n

This role will focus primarily on patients in the Detox Center, many of whom are struggling with substance use disorders and related mental health challenges. The LCSW will engage with a diverse population, providing compassionate care that meets both clinical and emotional needs. The position demands a strong understanding of the unique challenges faced by this clientele, along with a commitment to fostering a supportive and therapeutic environment.

Qualifications\n

To be successful in this position, candidates should possess the following qualifications:

• A Master’s degree in Social Work from an accredited institution.
