Key Responsibilities\n

• Conduct ongoing assessments to evaluate clients' needs and tailor treatment plans accordingly.

• Facilitate open communication among supervisors, direct care staff, families, and other stakeholders regarding client progress and treatment outcomes.

• Develop and propose schedules for appointments, home visits, and progress observations that align with client needs.

• Coordinate and perform in-home visits or virtual consultations for comprehensive assessments, including chart reviews and data analysis.

• Implement and monitor the effectiveness of treatment plans, ensuring adherence to established goals.

• Actively participate in interdisciplinary team meetings and contribute to collaborative care discussions.

Compensation and Benefits\n

The salary range for this position is competitive, offering between USD 90,000 and 100,000 annually, commensurate with experience and caseload availability.
