Key Responsibilities\n

As a Licensed Clinical Professional Counselor (LCPC), your responsibilities will include:

• Assisting the primary care provider in identifying, treating, and managing mental health and psychosocial issues, functioning as a vital member of the healthcare team.

• Conducting comprehensive patient intakes that emphasize diagnostic assessments and functional evaluations, followed by making informed recommendations to the primary care provider regarding patient treatment plans.
\n
• Providing brief, focused interventions to patients facing mental health challenges, ensuring that care is tailored to their unique needs.
\n
• Collaborating with primary care providers to determine which patients may benefit from continued care within the primary setting versus those that require specialty mental health referrals.
\n
• Implementing follow-up evaluations to monitor patient progress and adjust treatment strategies as necessary.
\n
• Developing relapse prevention plans and aiding patients in maintaining stable functioning post-treatment.
\n
• Identifying at-risk patients and proactively creating strategies to mitigate the worsening of their conditions.
\n
• Coordinating health service deliveries related to behavioral health needs, while linking patients with external treatment resources as permitted.
\n
• Documenting patient progress and developing treatment charts that reflect diagnostic information accurately.
\n
• Educating patients on self-management strategies and providing them with resources to actively participate in their own treatment journey.
\n
• Offering expertise to clinic management and colleagues regarding behavioral services to enhance program offerings and outcomes.
\n
• Supporting patient adherence to prescribed medical treatments initiated by primary care providers, including addressing potential side effects of medications.

• Upholding the Code of Ethical Conduct for Clinical Professionals during all interactions.

Patient Population / Client Focus\n

You will be engaging with a diverse range of patients who may present with complex mental health issues, including anxiety, depression, trauma, or chronic stress related to their social circumstances. Your role is critical in addressing the unique mental health needs of individuals within the community, thereby fostering resilience and overall well-being.

Qualifications\n

The successful candidate for this position will possess the following qualifications:

• A Master's Degree in Clinical Counseling or a related field.
\n
• Current licensure as a Licensed Clinical Professional Counselor (LCPC).
\n
• In-depth knowledge of mental health assessment methodologies, treatment planning, and psychotherapeutic approaches, particularly in family therapy and child psychotherapy.

• A thorough understanding of developmental psychopathology and DSM-IV diagnoses.
\n
• Strong assessment skills to evaluate patients’ individual needs and deliver effective care responses.
\n
• Ability to maintain collaborative relationships with professionals, patients, and families within an interdisciplinary team framework.

Preferred Experience\n

While not mandatory, candidates with the following experience will be favorably considered:

• Prior experience working in a Federally Qualified Health Center (FQHC) or similar community health setting.
