Key Responsibilities\n

• Deliver comprehensive psychosocial assessments of clients, creating individualized treatment plans in collaboration with healthcare team members, ensuring that plans include appropriate social work interventions and discharge strategies.

• Engage and cultivate relationships with key stakeholders in community resource groups, representing Redesign Counseling Hub at local meetings and committees aimed at addressing social determinants of health impacting our patient population.

• Provide therapeutic counseling and crisis intervention, utilizing advanced assessment skills to treat emotional and behavioral health concerns in a timely and effective manner.

• Maintain ongoing communication with healthcare professionals, including physicians and nurses, to relay patient progress, emerging needs, and any concerns that may arise during treatment.

• Ensure the maintenance of accurate clinical documentation, including progress notes and treatment records, in adherence with department policies and regulations.
