Key Responsibilities\n

• Conduct comprehensive psychosocial assessments through detailed interviews, noteworthy observations, and thorough examinations of medical records to ascertain the emotional, social, and environmental factors influencing patients’ recovery trajectories.

• Provide empathetic counseling and robust support to patients and their families, helping them navigate adjustment issues that arise from rehabilitation and addressing their emotional wellbeing.

• Work collaboratively with other members of the rehabilitation team to formulate individualized treatment plans that holistically consider patients' psychosocial requirements, thereby improving their outcomes.

• Guide patients in accessing vital community resources such as support groups, vocational rehabilitation services, or housing assistance, ensuring they have access to the tools needed for a successful reintegration into society.

• Offer ongoing support and education to both patients and their families regarding available community resources and post-rehabilitation services, empowering them to continue their journeys toward recovery.

• Play an active role in discharge planning, facilitating smooth transitions from the detox center to home or alternative care environments, coordinating ongoing support as necessary.
